Original uncertainty examples That care homes have more knowledge of how to cope with the ever changing condition. ~ Why do older people nearing the end of their life get discharged from hospital to rehabilatation centres, care homes or carers at home when all they really need is Hospice care at home 24 hours? ~ Care and support needs to be in one department to manage all the care, equipment and support patient and families will need.
